All pre-participation forms are completed using an online database. Please click on the link to "Pre-Participation Forms" below to see step-by-step instructions on how to complete these forms. Additionally, we need a CLEAR picture of the front AND back of your medical insurance card. NOTE: All student-athletes are now required to provide proof of their Sickle Cell Trait status before they can begin athletic participation. This information can be found with your local family physician or a test can be paid for at our on-campus Student Health Center.
