Consent to Release Medical Information Form
Authorize medical professionals to release your medical information to the Sports Medicine staff at Valdosta State University, and authorize the Sports Medicine staff at Valdosta State University to share your medical information with a physician.

Drug Testing Policy
By signing this form, you are consenting to participate in Valdosta State University Drug Testing Progam, a requirement to participate in VSU Athletic programs. This document contains the Valdosta State University Department of Intercollegiate Athletics Alcohol, Substance Abuse and Drug Screening/Testing Policies and Procedures.
